Forms & ID's

Complete Intake Forms

Before your appointment, please complete our  Intake Forms online. Alternatively, you may download and print them.

Bring A Valid Photo ID

By law, we are required to have proof of your identity (Driver's License, work ID, etc.). This will be copied into your private medical records. 

Payments

Consultation Fee

A consultation fee of $100 is required to reserve your appointment. Fees are redeemable for any treatments that are performed.

In-Office Treatments 

For in-office treatments, full payment is due at the time of service.

Surgery Center Procedure

For surgery center procedures, full payment of surgeon, facility, and anesthesia fees is due 30 days prior to the scheduled date of the procedure.

Payment Methods

We accept cash, MasterCard, Visa, Discover and American Express. Checks are only accepted for surgery center procedures.

No-Shows & Cancellations

In-Office Appointments

We charge a $100 reservation fee for consultations and treatments, which may be applied as credit to a future service or product. An additional $50 fee, which cannot be used as credit, will be assessed to any patient who cancels/reschedules within 2 days of their scheduled appointment OR who require rescheduling because they are more than 15 minutes late to an appointment.

Surgery Center Procedures

For surgery center procedures, if cancelling/rescheduling greater than 30 days prior to surgery, an additional $500 fee will be assessed. If less than 30 days prior to surgery, then a $1,000 fee. If less than 15 days prior to surgery, then a $2,000 fee.

Late Arrivals

The practice reserves the right to reschedule any appointment to which the patient is more than 15 minutes late.

Communication

Phone or Email?

Please provide a number or email upon which you feel comfortable receiving a private message. Email is convenient, but keep in mind that it not as secure as telephone.

Emergencies

We do not use e-mail for emergencies or situations needing immediate or urgent attention.

Medical Records

All calls or e-mails may be included in your medical records. At any time, you have the right to request medical communication via alternative means.
