Payments
Consultation Fee
A $100 consultation fee is required to reserve your in person consultation.
This fee may be applied toward a surgical or nonsurgical procedure if you move forward with treatment.
Payment Policy
Payment is due in full at the time of service for all in office treatments.
Surgery Procedures
For surgery procedures, all surgeon, facility, and anesthesia fees are due 30 days before your scheduled surgery date.

We accept cash, MasterCard, Visa, Discover, and American Express.

In-Office Appointments
A $100 reservation fee is required to schedule consultations and treatments. This fee may be applied as credit toward a future service or product.
An additional $50 fee will be charged if you cancel or reschedule within 2 days of your appointment, or if you arrive more than 15 minutes late and need to be rescheduled.
*This $50 fee is non-creditable and cannot be applied toward future services or products.

Surgery
Procedures
If you cancel or reschedule more than 30 days before your surgery date, a $500 fee will be charged.
If you cancel or reschedule within 30 days of your surgery date, the fee is
$1,000.
If you cancel or reschedule within 15 days of your surgery date, the fee is $2,000.

Arrivals
Patients who arrive more than 15 minutes late may need to have their appointment rescheduled at the discretion of the practice.
Communication
Phone or Email?
Please provide a phone number or email address where you are comfortable receiving private messages. Email is convenient, but please note it is not as secure as telephone communication.
Emergencies
Please do not use email for emergencies or situations requiring immediate or urgent attention.
Medical Records
Phone calls and emails may become part of your medical record. You may request communication through alternative methods at any time.
