Thank you for choosing us. We are dedicated to providing the best professional advice, care and treatment obtainable. Please review some of our office policies below.

Preparing for your visit

  • Complete the intake forms by downloading and printing them from this site, or we will provide you copies to complete at the office.
  • Bring a valid photo ID (Driver’s License, work ID badge, etc). By law, we are required to have proof of your identity, and this will be copied into your private medical records.
  • If a medical visit, bring your all your current medical insurance cards.

Communicating with the office

  • Please let us know your preferred method of communication with our office: phone, email, or either.
  • Please provide a number or email upon which you feel comfortable receiving a private message.
  • Email is convenient, but keep in mind that it not as secure as telephone. We do not use e-mail for emergencies or for situations needing immediate or urgent attention.
  • All calls or e-mails may be included in your medical record(s). At any time, you have the right to request medical communication via alternative means.

Cosmetic consultations & treatments

  • The consultation fee of $100 and is required to reserve your appointment. Fees are redeemable with any treatments that are performed.
  • For in-office treatments, full payment is due at the time of service.
  • For surgery center procedures, full payment of surgeon, facility, and anesthesia fees is due 2 weeks prior to the scheduled date of the procedure.
  • We accept cash, MasterCard, Visa, and CareCredit. A financing fee is charged for CareCredit transactions. Checks are only accepted for surgery center procedures.

Medical consultations & treatments

  • Fees are billed to your insurance company. We accept most major insurance plans.
  • You are responsible for all co-pays, cost-shares and deductibles the day of the visit.
  • If you do not have your insurance card, full payment may be due at the time of service.

Fees for no-show & cancellation

  • For consultations and in-office treatments, $100 will be charged for no-shows or cancellations that occur within 24 hours of the scheduled appointment.
  • For surgery center procedures, we charge $250 for rescheduling and $500 for cancellation, if less than 2 weeks prior to surgery.
  • The practice reserves the right to reschedule any appointment to which the patient is more than 15 minutes late.
