Lengthening The Nose With Rhinoplasty
Lengthening the nose is one of the more complex rhinoplasty procedures. Most people interested in nose lengthening rhinoplasty have either an underprojected, upturned or saddle nose.
How do you add length to the nose?
Cartilage is the primary way rhinoplasty surgeons "build up" a nose. Cartilage can be taken from a few different areas: the septum, bowl of the ear or the rib depending on the strength of cartilage needed for the specific procedure. Rib cartilage is the strongest. Preferably, this would be the patient's own but donor cartilage may also be used.

Underprojected NoseList Item 1
An underprojected nose nose may appear flat from the side. The most common way to correct this with surgery is to place a cartilage nasal tip graft that projects farther than the existing tip cartilages.

Upturned NoseList Item 2
With an upturned nasal tip, the nostril openings are much too visible from the front and the nose may appear too short overall. During surgery, the nasal tip may be pushed downward by using cartilage grafts the extend the septum and tip complex to a lower position. A cartilage graft secured on the nasal tip may also lower the apparent position of the tip and add more projection.

Saddle NoseList Item 3
A saddle nose is a term used for a nose that has a low bridge or indented profile. This appearance is caused by insufficient cartilage and bone in the nasal bridge. It may be congenital, an ethnic trait or the result of trauma or a previous surgery. Below is an example of a patient who opted for rib cartilage to add structure to her nasal bridge.
